Contribution Statement
Funds received by the Louisiana Diabetes
Foundation from grantors and contributors are exempt from
federal income tax under section 501(c)(3) of the Internal Revenue
Code. If you so indicate, the Louisiana
Diabetes Foundation will distribute funds to individual and/or
corporate diabetes reseachers, Diabetes
America, or the National Diabetes Alliance. Diabetes America
is a division of the Louisiana Diabetes
Foundation, but may receive support from others. The National
Diabetes Alliance is a partnership of
independent diabetes organizations in America that individually
may, or may not be, tax exempt. In all cases,
funds distributed to any of these recipients will remain dedicated
to the purposes of the Louisiana Diabetes
Foundation and are so restricted when distributed at your direction.
